Job Summary:
The Antimicrobial Stewardship Pharmacist will be an integral member of the Pharmacy team, a key player in establishing initiatives that foster improved antimicrobial use, lower antimicrobial resistance, support patient safety and cost-savings across all sites of the Niagara Health System. The successful incumbent will work with the Pharmacy and Therapeutics Committee and laboratory liaisons to establish guidelines that ensure appropriate antimicrobial therapy including: antibiotic prophylaxis, establishing guidance for empiric antimicrobial therapy, optimizing antimicrobial therapy associated with a definite diagnosis, optimizing antimicrobial dose and route of administration as well as duration of antimicrobial therapy, supporting annual publication of an antibiogram. Collaborative work with Physicians, the Department of Pharmacy Services, Infection Prevention and Control and the Quality and Education Department will be required to provide implementation strategies for initiatives including: education to hospital staff, prospective and retrospective evaluation of ongoing compliance and provide feedback establishing a formulary of antimicrobials.
The Antimicrobial Stewardship Pharmacist will assist in establishing an ongoing antimicrobial and drug surveillance reporting program under the guidance of the Pharmacy and Therapeutics Committee. Participation in antimicrobial utilization quality initiatives in conjunction with the hospital’s continuous quality improvement initiatives, and as leader, the responsibility for providing education and feedback to pharmacists and physicians regarding appropriate use of antimicrobial agents.
Qualifications:
· BSc Pharmacy degree with infectious disease training or the equivalent clinical experience.
· Current registration with the Ontario College of Pharmacists in part A of the register.
· Minimum of three years Hospital pharmacy experience.
· Flexible, self-directed and able to work effectively with minimal supervision.
· Demonstrated effective problem solving and critical thinking skills.
· Demonstrated proficient computer skills.
· Previous experience with the Pharmacy Meditech module preferred.
· Evidence of professional development and continuing education.
· Demonstrated organization and time management skills needed to handle a diversified and changing workload.
· Demonstrated leadership qualities.
· Demonstrated ability to adapt well to progressive change.
· Demonstrated ability to communicate and work effectively within a multi-disciplinary team.
· Flexible, self-directed and able to work effectively with minimal supervision.
