JOB TITLE: ANTIMICROBIAL STEWARDSHIP PHARMACIST
COMPETITION NUMBER: 11-572
STATUS: PERMANENT FULL TIME
DEPARTMENT: PHARMACY SERVICES
REPORTS TO: MANAGER, PHARMACY SERVICES
HOURS: DAYS; 7.5 HOUR DAYS
JOB SUMMARY:
The qualified candidate will work collaboratively with a multidisciplinary team to develop and implement the Antimicrobial Stewardship Program. As an integral member of the stewardship team, you will be a key leader in establishing initiatives that foster improved antimicrobial use, mitigate antimicrobial resistance, increase patient safety and yield cost savings across the organization. You will work closely with the Antimicrobial Stewardship Infectious Disease Physician and the Departments of Pharmacy and Infection Prevention and Control to develop implementation strategies for initiatives including education of hospital staff as well as prospective and retrospective evaluation of ongoing compliance. Developing and maintaining an expert level of knowledge of antimicrobials will be key to your function as a resource person, to provide feedback to pharmacists and prescribers on the appropriate use of antimicrobial agents.
QUALIFICATIONS:
Bachelor of Science in Pharmacy and/or Doctor of Pharmacy degree
Current, Part A pharmacy licensure with the Ontario College of Pharmacists
Minimum of 3 years experience in hospital pharmacy practice with demonstrated clinical leadership skills
Hospital Pharmacy Residency is preferred
Broad clinical knowledge and expertise in infectious diseases required
Previous experience in antimicrobial stewardship is preferred
Project management skills an asset
Excellent verbal and written communication, interpersonal, team management and organizational skills
Excellent problem solving skills
Demonstrated ability to take initiative, work independently and as a team member , in a past paced challenging environment
Good knowledge of Microsoft Office Word, Excel, PowerPoint, Outlook
Demonstrates behaviours consistent with York Central Hospitals Code of Conduct.
Proven attendance record.
Ability to perform the essential duties of the job.
