Position: Pharmacist

Site: General

Unit/Department: Inpatient Pharmacy

Reports To: Manager, Pharmacy Operation

Hours: 1.0 FTE, days/evenings, 8 hour shifts, including weekends

Status: Full Time Regular

Salary: $75,120.63 to $90,506.72

POSITION SUMMARY:

Contributes to overall patient care through involvement in the provision of direct patient care, medication assessment, medication order review and entry, patient medication safety, and drug distribution services in the inpatient setting, drug information and educational programs. All roles are essential in providing a safe and healthy environment for patients, staff, physicians, volunteers and others by ensuring that health and safety policies and regulations are followed.

MAJOR DUTIES AND RESPONSIBILITIES:

Provides patient-oriented clinical services ? medication order review, medication profiling, therapeutic drug monitoring, drug use evaluation, adverse drug reaction reporting, medication counselling, medication incident reporting and follow-up, warfarin dose management and medication history for medication reconciliation.
Maintains activities in all drug use control functions - drug distribution systems, computerized medication system, formulary system, inventory control system, barcoding system and drug procurement system.
Participate in the training and receive certification in IV admixture service, for the provision of efficient and safe distribution of IV medications service.
Participate in department quality improvement initiatives.
Participates in drug information and other educational activities within the department, on the nursing units and throughout the Hospital.
Responsible for the day-to-day activities of the pharmacy assistants/technicians.
Follows all Hospital and governmental (Federal and Provincial) rules and regulations set to control the dispensing to the general public and the distribution of medications, and to provide high quality services within the Hospital.
Responsible for working safely in accordance with the Occupational Health and Safety Act.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

Graduate from a recognized university Pharmacy Program (Bachelor level).
Licensed with the Ontario College of Pharmacists (Part A)
Hospital clinical pharmacy experience preferred

SKILLS REQUIRED:

Demonstrated knowledge and skills in pharmaceutical care, disease management and clinical pharmacology.
Demonstrated written and verbal communication skills.
Deductive reasoning.
Demonstrated pharmaceutical skills and techniques, e.g., sterile procedures.
Demonstrated skills in computerized pharmacy system.
Adaptability.
Demonstrated people skills in client/customer service.
Bilingualism in French/English an asset.
